At the time of service, we will estimate your insurance benefits and collect the portion not covered. We accept:

I understand that Dr. Austin Harper and his staff reserve appointment times specifically for me. I agree to confirm my appointment by phone or text at least 48 hours prior to my scheduled time. If I do not confirm my appointment within this timeframe, I understand that my appointment may be released to another patient on the waitlist.
If I confirm my appointment and fail to appear, or cancel the same day without appropriate circumstances (such as illness or emergency), this will be considered a missed appointment. In such cases, I understand that I will be required to pay a $75 rescheduling fee before booking another appointment. Once the fee is paid, I must schedule a new appointment for the same day or the following day. If no appointments are available within that timeframe, I understand that I will need to call back to schedule.
If a second missed appointment occurs under these same conditions, I understand that I may be dismissed from the practice.
Your insurance policy is a contract between you, your employer, and the insurance company — not Auburn Dental.
We will estimate your benefits to the best of our ability, but coverage is not guaranteed. Insurance estimates are based on the information provided by your insurance company and are not guarantees of payment. Any remaining balance after insurance processes your claim is the responsibility of the patient.
Payment for Restorative Treatment
For certain procedures, payment is collected in stages:
Patients receiving restorative or major treatment will be asked to review and sign a treatment plan agreement prior to scheduling.
Unpaid balances over 90 days are subject to a 1.5% monthly finance charge (18% annually).
If you haven’t received confirmation of payment within 60 days of service, please contact your insurance company or our office.
We reserve the right to discontinue treatment for accounts that remain unpaid or fail to meet payment terms.
